
1. Title. I (The Primary applicant) agree that the item(s) I am renting is/are the property of Video Paradiso.
2. Member Deposit. I (The Primary applicant) agree to leave a credit/debit card number on file.
3. Rental Period. Rentals are for 3 nights / 2 days. This means if I rent a video on Monday it is due back Thursday by 10am. All
rentals are due back by 10am.
4. Additional Rental Fee. I understand that I will be charged an Additional Day rental fee of $.99 (+Tax) each day/per film past the
rental due date.
5. Rental/Reservation Privileges. I (The Primary applicant) understand that I may rent a maximum of six (6) movies at any given
time. I am entitled to reserve movies up to fourteen (14) days in advance, however they are not guaranteed to be available for
rent.
6. Lost, Stolen, Damaged or Destroyed Videos. I (The Primary applicant) understand that items must be returned in the same condition
as when they were picked up, except for normal wear and tear. I understand that I will be held liable for any items, which are lost,
stolen, returned, damaged and/or destroyed, for full replacement value of the item(s).
7. Forfeiture of Deposit/Chargebacks. I (the Primary applicant) understand that any rented item not returned to Video Paradiso
within fourteen (14) days after the rental may be converted into a sale. I am then responsible for the full replacement price plus
all late fees incurred/accrued. I shall then become owner of the item(s), and I authorize Video Paradiso to charge my credit/debit
card, I have left for Video Paradiso’s protection, account for payment. I also understand that, should funds be unavailable from my
credit/debit card, Video Paradiso may then turn my account over to a credit agency, which will seek compensation and, potentially,
affect negatively my credit rating.
8. Outstanding Balances. I (The Primary applicant) understand that I must keep my membership account balance paid in full at all
times. I understand that any balances not paid within thirty (30) days of the last transaction shall be charged to my credit/debit
card for payment. Should funds be unavailable from my said card, Video Paradiso may then turn my account over to a credit agency,
which will seek compensation and, potentially, affect negatively my credit rating.
9. Defective Films. I (The Primary applicant) understand that if I rent an item that is defective (an item which will not play
properly in my VCR/DVD player) that I am to call Video Paradiso promptly with the problem (Video Paradiso will check the problem using
their equipment. If Video Paradiso replicates the problem your account will be credited for the defective item). I understand that
the video is still due by the due date listed and if it is not returned on time that I will owe all late charges accrued. Further, I
understand that Video Paradiso is not liable for any damages my player incurs while playing a Video Paradiso rental.
10. Responsibility. I (The Primary applicant) understand that I am responsible for all financial fees incurred and all items rented
by me and by all others linked to my membership account.
